It is in the interest of the teacher candidates to complete a Work/Education Placement Agreement for every placement. In the event of injury, a teacher candidate can claim Workplace Safety and Insurance coverage only if this form is completed by both parties (teacher candidates and host schools/community organizations). Seminar tutorial leaders should not sign this form. It should be signed by the school principals. Once completed, teacher candidates should keep a copy for themselves and provide a copy to their Consecutive Site Course Directors or Concurrent (and Part-time Consecutive) Seminar Leaders.
